HHH Choices is a managed long-term care program for people in The Bronx that are on Medicaid.
Our program was created in 1998 and provides many services for the elderly and people who have
physical problems, have chronic illness, or are disabled.
Our Nurse Care Managers oversee all care
given.
Our goal is to help our members stay at home in the community and help them live as
independently as they can.
Since we began, we have continued to grow and provide Bronx
residents with community based services.
Some of the services we provide are: Care Management
by a Registered Nurse; Nursing Visits & Aide Services; Non-Emergent Transportation; Nursing
Home Care; Dental, Vision, & Podiatry (foot care); Hearing Aides, Medical equipment, Orthotics/
Prosthetics; Physical, Speech, & Occupational Therapy; Social Work & Nutritional Counseling;
Adult Day Care & Meals on Wheels; Emergency Response Systems and More.
